Free Clinical Compliance Requirements Checklist for Program Directors

For program directors and clinical coordinators. Nine program versions, free.

Every clinical site sends its own list, and every program ends up rebuilding the same checklist each term. This one is already built: the requirements health science programs most often collect before clinicals, with a version for each of nine programs. Use it to brief incoming students, compare against your affiliation agreements, or audit a cohort before rotations start.

What’s on every checklist

  • Immunizations and titers: MMR, varicella, hepatitis B, Tdap, influenza, and any vaccine a site adds.
  • TB screening: two-step skin test or IGRA blood test, and the renewal schedule your sites set.
  • CPR / BLS: healthcare-provider card, tracked to its expiration.
  • Background check: name-based, with healthcare sanctions screening where hospitals require it.
  • Drug screen: commonly a 10-panel urine screen before the first placement.
  • N95 fit test: where a site requires respirators.
  • Health insurance: proof of coverage, if your sites require it.
  • Training: HIPAA, OSHA bloodborne pathogens and other site safety modules.
  • Site orientation forms: each site’s orientation, confidentiality agreement and acknowledgments.

Each line has a box to check and space for the completed and expiration dates. It prints cleanly on letter paper.

Versions for nine programs

Each version adds items that are common for that program, marked “Commonly required; confirm with your clinical sites”:

  • Nursing: Physical exam or health clearance form; Clinical site access forms; Professional liability coverage.
  • Radiography: Radiation monitoring badge (dosimeter); Pregnancy policy acknowledgment; MRI safety screening form.
  • Surgical Technology: Operating room orientation; Latex allergy or sensitivity disclosure; Case log set up.
  • Respiratory Therapy: N95 fit test for each model the site uses; Additional TB screening; Physical exam or health clearance form.
  • Medical Laboratory Technology: Biohazard and lab safety training; Meningococcal vaccines; State trainee license.
  • PTA and OTA: Technical standards acknowledgment; Site requirements reviewed before each placement; Extra clearances for pediatric or school sites.
  • Dental Hygiene: Continuous healthcare-provider CPR/AED; Radiation hygiene and hazardous materials policies; Immunization declination forms.
  • EMS and Paramedic: Hospital and field requirements tracked separately; Field agency forms and orientation; Physical exam or health clearance form.
  • Medical Assisting: Practicum site agreement and supervisor contact; Competency check-offs; Physical exam or health clearance form.

Traps that get uploads rejected

  • Partial uploads: page 2 of a lab report missing.
  • The wrong titer: an IgM result where IgG is required. See titer testing explained.
  • TB without the reading: a skin-test placement uploaded with no read date and result. See TB testing requirements.
  • Expired cards: a CPR card that lapses during the term.
  • Old fit tests: an N95 fit test older than the site allows.

Want it tracked instead of on paper?

Our clinical compliance tracking portal tracks every item on the checklist: every upload reviewed within 24 hours with a note that says exactly what to fix, and email and text reminders 30, 14 and 7 days before anything expires. $30 per student, one time. We also run the drug screens, name-based background checks and N95 fit testing. Clinical site coordinators can see how records reach them on For clinical sites.

Frequently asked questions

Is the checklist free?

Yes. Fill in the form and all nine program checklists open as soon as you submit.

Does requesting the checklist create a demo account?

No. The checklist request only sends us your name, title, school, program and email. If you want a demo login later, you can ask for one separately.

Can we edit the checklist for our program?

Yes. It is a starting list. Your clinical sites, affiliation agreements, accreditor and state can add items, so edit it to match the rules your students actually have to meet.

What are the most common reasons student uploads get rejected?

Partial uploads (page 2 of a lab report missing), an IgM titer where IgG is required, a TB placement uploaded without the reading, an expired CPR card, and a fit test older than a site allows.

Is this legal or medical advice?

No. The checklists list requirements that health science programs commonly collect. Items that vary most by program are marked “Commonly required; confirm with your clinical sites.”

Do you take fingerprints?

No. Our background checks are name-based. If a state agency or clinical site requires fingerprints, students use the provider it names, such as Fieldprint or IdentoGO, or their local police department.

The checklists describe requirements health science programs commonly collect. Requirements vary by school, state, accreditor and clinical site; confirm with yours. Fieldprint and IdentoGO are trademarks of their respective owners. Nothing on this page is legal or medical advice.
